As a newcomer to this group I was pleasantly overwhelmed by the numerous
(about 20) and helpful responses I received to my inquiry about creating a
needs assessment questionnaire for my small hospital library.  Thanks so
much to everyone who responded.  I will definitely be able to use a lot of
the suggestions.

One list member emailed and asked me to summarize the responses.  However,
most of the responses included attachments of sample questionnaires and/or
rather lengthy replies.  Since List Mom just wrote about keeping messages
brief, I will offer instead to forward all of the replies I received to
anyone who contacts me directly.
