Hi Tom,
We give a verbal preliminary to the referring doc and keep the patient only on symptomatic carotid patients with 70-99% stenosis.  Also for anyone who experiences a TIA during scanning.  Asymptomatic 70-99% are processed as usual.  
We actually had a man the other day with one month history of left hand/arm tingling.  He had a 373/137 cm/sec internal stenosis on the right side.  He experienced the tingling during the scanning.  We paged his referring doctor who had him go to the ER.
