Take a look at youtube.com. There should be plenty of accident scenarios to choose from, for example the QVC ladder video.

Hello, 
I have been following this list for just about four months and this is my first time posting. 
I am preparing to do a training session on Accident Investigation for Supervisors and am looking for good material. 
Possibly a scenario or video where participants can conduct an investigation from what I show or present.
