Has anyone tried the pressure gauge for the patient to blow into for a good valsalva? The Merrill Spencer lab uses it for TCD PFO studies?
I have some patients blow into a tubing, O2 tubing on the end of a syringe and that worked well. Don't use it all the time, just difficult patients.

This works every time. Tell your female patients to “take in a deep breath, hold it, now don’t breathe, and make a big pregnant belly, ok breath and relax”.  I do the same with the men only I tell them to make a big beer belly. It helps to  start with them in a reverse Trendelenberg.
