It helps to track the cards in a visible way so that employees get the
feedback. I did not tie it to incentives. You can show the area of
concern as information and actions taken; done well you have
documentation of improvement and possibly trends.
We also had a small pocket size card for voluntary observations. Two
people chose an area for observation and one would record the activity
the other was observed. Limited to five areas worth 20 pts - driving
skills, tool/equipment operation what ever they choose or what the focus
is for the time period (say the topic of the safety talk voluntary
observation the week between tool box talks) Guys usually had an area
for improvement and it would create learning opportunities.

Hello All,
We are considering using the safety observation process on a few of our
jobsites as a way to implement a safety incentive program. Our thought
is to
have the subcontractor tradesman fill out a safety obervation card for
good
and bad observations and turn them into a box. We would collect the
cards
daily and make sure the observations were followed up on (or better yet
the
tradeperson would have to follow up to even turn in the card) and then
pull
cards out of a hat on a weekly/monthly basis and provide rewards.
Anyone
already do this or have a good form I can use?
