I don&apos;t see it a particularly onerous task to attend the meetings, although they are at 7 AM, which is not when I&apos;m the brightest. I seldom have any special assignment, although occasionally they want a search done or some message delivered to one of our docs.&lt;br&gt;We approve all the activities, and each member is supposed to audit several activities each year, which I do.   <content type='html'>Helena,&lt;br&gt;You bring up a great point with your example.&lt;br&gt;&lt;br&gt;More broadly, shouldn&apos;t NLM be indexing, and more importantly, making phrases out of ALL government health-related division, department, etc. names, products like surveys, tools, etc.? Why do we have to resort to creating such obtuse search strings that ultimately still miss a lot anyway? Can&apos;t they crowd source the process so those in the field can suggest and vote, to expedite the creation of official phrases that can be searched? Of course they are inadequately staffed, it is understandable it takes a while for the staff to accomplish the discovery [...] </content>

  <content type='html'>Tom makes a very valid point-- you have to search using both MeSH and title/abstract terms. However, PM also does that funky thing of trying to figure out what you **really** meant, then reworking your search to include terms that you probably would have added if you&apos;d only know.&lt;br&gt;&lt;br&gt;My favorite demo when searching PubMed is NHANES. Try it-- enter &quot;nhanes&quot;, get those 22K hits. And look at the search details: &quot;nutrition surveys&quot;[MeSH Terms] OR (&quot;nutrition&quot;[All Fields] AND &quot;surveys&quot;[All Fields]) OR &quot;nutrition surveys&quot;[All Fields] OR &quot;nhanes&quot;[All Fields] [...] </content>

I tell them they might want to check it when they get very little retrieval or thousands of hits that are close to what they want, but don&apos;t nail the topic. This is after they have checked the search details and the results have mapped to the topics they want. I mention I use Mesh when I am not familiar with a topic.   <content type='html'>Technology and websites are evolving so quickly that we also have to be on&lt;br&gt;our toes to see how changes are happening (referring to what worked with&lt;br&gt;mesh mapping or did not)&lt;br&gt;&lt;br&gt;I stress the importance of MeSH when I teach. Of course it will drop some&lt;br&gt;references and also the not-yet-indexed ones. And yet, it picks up articles&lt;br&gt;where title does not contain the word and there is no abstract. But people&lt;br&gt;need to understand why that has been picked up. My favourite examples are&lt;br&gt;two titles [...] </content>

I remember a NLM Medline workshop where we had to come up with searching for the EMI scanner literature - the joint project yielded about 3 pages of variations on controlled vocabulary. I also remember spending 3 blasphemous days trying to search for wrinkles without text words - it turned out it was under FACE. I do remember the AIDS problem &quot;previously healthy&quot; for example. I do miss string searching though. [...] </content>

  <content type='html'>IMHO, the gold standard is to use both MeSH and text words if you want to be comprehensive. Reasons:&lt;br&gt;&lt;br&gt;1. Weak cross-referencing/mapping of MeSH terms in some topical areas.&lt;br&gt;2. Brand new articles not yet indexed with MeSH.&lt;br&gt;3. Variant terminology, including slang.&lt;br&gt;4. New concepts/terminologies can take time to percolate into MeSH. Old searchers like me remember what it was like to try to search AIDS in the first couple of years of that disease&apos;s identification. This was compounded by the U.S. term vs. the French term for the organism (HIV vs. HTLV). The concept of Lung Injury was [...] </content>

  <content type='html'>I view MeSH this way.   If the gold standard for pertinent articles is hand searching journals, then the silver standard is a search using Major MeSH.  The next best thing is MeSH, not Major.   The most time-consuming thing is, in my opinion, the keyword search.   I know we see research articles where the author identifies a set of keywords used, and that has its place when the author wants to pre-verify if any article ever on a certain topic has been written.   For locating several to a few pertinent articles for a client, though, I find that Major MeSH is best, perhaps followed and combined [...] </content>

I called NLM and&lt;br&gt;expressed my disbelief that it didn&apos;t map. The other ones were critical&lt;br&gt;path/care map/care planning that was not a term in the late 1990s.&lt;br&gt;&lt;br&gt;There is always a lag between when something becomes popular and when it&lt;br&gt;becomes a MeSH term, but that doesn&apos;t mean we shouldn&apos;t use MeSH!!!   <content type='html'>I noticed our original poster is not a native English speaker. I think a lot of the confusion around MeSH and Boolean simply don&apos;t make sense to non-English speakers. In French (for example), a camion is a truck, whether you&apos;re in Belgium or Mali or Haiti or Quebec. In English a lorry is a truck and everybody knows you&apos;re talking about the same thing whether you&apos;re from Australia, Canada, Scotland or the US. Or do you not bother?&lt;br&gt;&lt;br&gt;I will admit it offends my sense of order in the universe to see a search that is just a bunch of keywords strung together without even an AND between terms but I realize that is a pretty flimsy reason to change someone else&apos;s search habits. How much does the lack of an AND mess up the automatic term mapping? [...] </content>

  <content type='html'>I always try to use a combination of MeSH and keywords, especially when retrieving unindexed/recent items is important. There are definitely common topics for which automatic term mapping doesn&apos;t work the way you might expect, such as with common acronyms like HRT (doesn&apos;t map), ADHD maps but ADD doesn&apos;t, &quot;plan b&quot; won&apos;t map to levonorgestrel if you put it in quotes but it will map if you don&apos;t, etc. If you&apos;re going to rely on ATM, I&apos;d recommend checking each term in the search details to see if it&apos;s actually doing what you expect. The difference between &quot;high blood pressure AND salt&quot; (yes, I said it was unsophisticated, but you know how some people search...) and hypertension/diet therapy/major heading AND sodium chloride is pretty startling.   <content type='html'>Personally, I think librarians and super duper savvy searchers are the only ones who really use MeSH because of the various reasons people have listed so far. I mention MeSH and explain it a bit, but I also know that the average user is going to keyword the heck out of things anyway because that is how Google raised us. That is why I make sure I always point out the Search Details box to show them how the system is thinking so they can adjust their search accordingly if the system misunderstands our intentions. </content>

The tree structures are invaluable when I want to make sure that one explodes and includes my concepts. Gynecologic Surgery is an entry term for Gynecologic Surgical Procedures, which includes hysterectomy, colposcopy, etc. Good info, here! [...] </content>

  <content type='html'>I agree with Julie and would add that you should check the details to see&lt;br&gt;what MeSH terms PubMed has automatically matched your keywords to.&lt;br&gt;Sometimes it is great but sometimes it takes you in a direction you do not&lt;br&gt;want to go. Do a search using *Nursing* as a keyword and check the details.&lt;br&gt;Are all the MeSH terms the topic you wanted to search for? Do a search for&lt;br&gt;&quot;insulin pump,&quot; note the number of results. Then do a search for &quot;Insulin&lt;br&gt;Infusion Systems&quot;[Mesh], note the number of results. Then run (&quot;insulin&lt;br&gt;pump&quot; OR &quot;Insulin Infusion Systems&quot;[Mesh]), note [...] </content>

  <content type='html'>I would argue that automatic term mapping is what makes PubMed so much better than other search engines.&lt;br&gt;&lt;br&gt;In a lot of searches, I find that my basic search results are better than my MeSH search results, but there are some for which MeSH is absolutely essential.&lt;br&gt;&lt;br&gt;To me, one of the biggest disadvantages of MeSH is that the results won&apos;t include the very newest articles (those that are not indexed for Medline yet).   <content type='html'>I am old-school, admittedly (been a librarian nearly 20 years), but I find&lt;br&gt;MeSH extremely, extremely helpful. Controlled vocabulary has some real&lt;br&gt;benefits. It is hard to teach sometimes, but well worth the effort.&lt;br&gt;Controlled vocabulary is what makes PubMed so much better than other search&lt;br&gt;engines.&lt;br&gt;&lt;br&gt;Off of my soapbox now!&lt;br&gt;&lt;br&gt;I manage a taxonomy at work. The new criteria reflect the Institute of Medicine&apos;s definition of a clinical practice guideline provided in its 2011 standards-setting publication, Clinical Practice Guidelines We Can Trust.
