My sister bought a Kindle for her thinking the ability to enlarge the print would help, but since the background is grey it is still difficult for her to read. I'm on our local (it's a combined city-wide program) CME committee. We (all of us) mostly sit around rendering opinions. I don't see it a particularly onerous task to attend the meetings, although they are at 7 AM, which is not when I'm the brightest. I seldom have any special assignment, although occasionally they want a search done or some message delivered to one of our docs.
We approve all the activities, and each member is supposed to audit several activities each year, which I do. That's just sitting through one of the tumor boards or a section meeting and [...]

Our physical library will be closing July 26th. There will be a virtual library & I will stay on to administer. My co-worker & I have been delegated to figure out what to do with all of our print journals, A/V & books. Right now we have more questions than answers. Is there anyone out there who has gone from a physical library to a virtual library & what is the online database you use? You bring up a great point with your example.

More broadly, shouldn't NLM be indexing, and more importantly, making phrases out of ALL government health-related division, department, etc. names, products like surveys, tools, etc.? Why do we have to resort to creating such obtuse search strings that ultimately still miss a lot anyway? Can't they crowd source the process so those in the field can suggest and vote, to expedite the creation of official phrases that can be searched? Of course they are inadequately staffed, it is understandable it takes a while for the staff to accomplish the discovery [...]

Tom makes a very valid point-- you have to search using both MeSH and title/abstract terms. However, PM also does that funky thing of trying to figure out what you **really** meant, then reworking your search to include terms that you probably would have added if you'd only know.

My favorite demo when searching PubMed is NHANES. Try it-- enter "nhanes", get those 22K hits. And look at the search details: "nutrition surveys"[MeSH Terms] OR ("nutrition"[All Fields] AND "surveys"[All Fields]) OR "nutrition surveys"[All Fields] OR "nhanes"[All Fields] [...]

During orientation, I mention there's a database called Mesh. I tell them they might want to check it when they get very little retrieval or thousands of hits that are close to what they want, but don't nail the topic. This is after they have checked the search details and the results have mapped to the topics they want. I mention I use Mesh when I am not familiar with a topic. (Basically, I am happy when they do a search in PubMed instead of just relying on Google or UpToDate). [...]

Technology and websites are evolving so quickly that we also have to be on
our toes to see how changes are happening (referring to what worked with
mesh mapping or did not)

I stress the importance of MeSH when I teach. Of course it will drop some
references and also the not-yet-indexed ones. And yet, it picks up articles
where title does not contain the word and there is no abstract. But people
need to understand why that has been picked up. My favourite examples are

Hello, everyone. I'd like your opinion, please. NOOK is having a great sale right now, & I'm wondering if the NOOK HD+ 9", 32 gig reader/tablet is better than the Kindle Fire for healthcare professionals.

I'd like to be able to read textbooks and leisure books, borrow library ebooks, watch YouTube videos, perhaps use Hulu/NetFlix/DVD digital copy, apps for my kindergardener, apps for elderly & pediatric patients, and maybe hook it up to a TV. Eons ago, indeed. I started searching in 1972. No text words, no abstracts. I remember a NLM Medline workshop where we had to come up with searching for the EMI scanner literature - the joint project yielded about 3 pages of variations on controlled vocabulary. I also remember spending 3 blasphemous days trying to search for wrinkles without text words - it turned out it was under FACE. I do remember the AIDS problem "previously healthy" for example. I do miss string searching though. [...]

IMHO, the gold standard is to use both MeSH and text words if you want to be comprehensive. Reasons:

1. Weak cross-referencing/mapping of MeSH terms in some topical areas.
2. Brand new articles not yet indexed with MeSH.
3. Variant terminology, including slang.
4. New concepts/terminologies can take time to percolate into MeSH. Old searchers like me remember what it was like to try to search AIDS in the first couple of years of that disease's identification. This was compounded by the U.S. term vs. the French term for the organism (HIV vs. HTLV). The concept of Lung Injury was [...]

I view MeSH this way. If the gold standard for pertinent articles is hand searching journals, then the silver standard is a search using Major MeSH. The next best thing is MeSH, not Major. The most time-consuming thing is, in my opinion, the keyword search. I know we see research articles where the author identifies a set of keywords used, and that has its place when the author wants to pre-verify if any article ever on a certain topic has been written. For locating several to a few pertinent articles for a client, though, I find that Major MeSH is best, perhaps followed and combined [...]

We transitioned earlier this year to ClinicalKey from MD Consult. We use a proxy for site-wide access on- and off-campus, so there was no trouble for folks to get into the new database. However, if someone had set up a personal account for MDC, their credentials did not migrate to ClinicalKey. They had to set up a new personal account. [...]

In 1989 a researcher asked for a search on raf. It wasn't a MESH heading, so I had to use text words. Once citation was totally irrelevant - it talked about the Royal Air Force. And THAT's the example I give for why text words aren't always the best bet. [...]

My greatest example is that electronic medical records/electronic health
records was not a MeSH term until 2006 or later. I called NLM and
expressed my disbelief that it didn't map. The other ones were critical
path/care map/care planning that was not a term in the late 1990s.

There is always a lag between when something becomes popular and when it
becomes a MeSH term, but that doesn't mean we shouldn't use MeSH!!! [...]

I am SO out of my depth here I don't even know where to start. IT
department wants to develop some way of measuring the value of their
projects, as a way of both establishing the value of what they do, but also
as a way of setting priorities, evaluating success, etc. This is NOT
exactly "ROI" as in "should we buy X database / tool / app / software."
The only thing I can compare it to in my world is establishing value of
various library functions, which I get pretty well, but since I am not a

In addition to points already made, I frequently use MeSH for backtracking newer drugs. Entry terms under the drug name include trade names, synonyms (sometimes from other countries), drug registry numbers, and - most important to me - the alphanumeric that was used to identify the drug in early research studies. Based on my experience the value of MeSH is still very important for
searching, but indexing in many cases is not describing the context of
articles - is simple bad.
Very often I find articles through Google that I didn't get from my
Medline search, check the index terms and as an ex-indexer can't figure
out why some MeSH terms were attached to the long list of MeSH terms,
but the terms most important for this article are missing. [...]

I noticed our original poster is not a native English speaker. I think a lot of the confusion around MeSH and Boolean simply don't make sense to non-English speakers. In French (for example), a camion is a truck, whether you're in Belgium or Mali or Haiti or Quebec. In English a lorry is a truck and everybody knows you're talking about the same thing whether you're from Australia, Canada, Scotland or the US. Yes, Boolean operators are the first things that I explain when teaching searching for any database.

It seems easy for them to understand if you use the shopping example: you go to the store looking for a shirt AND it has to be blue AND it has to have ruffles AND it has to have short sleeves AND it has to have a pink stripe, etc.
Also, you want to buy something green AND it can be a shirt OR shoes OR pants OR ribbons, etc. [...]

As a side question, how do you convince novice PubMed searchers to use Boolean operators? Or do you not bother?

I will admit it offends my sense of order in the universe to see a search that is just a bunch of keywords strung together without even an AND between terms but I realize that is a pretty flimsy reason to change someone else's search habits. How much does the lack of an AND mess up the automatic term mapping? [...]

I always try to use a combination of MeSH and keywords, especially when retrieving unindexed/recent items is important. There are definitely common topics for which automatic term mapping doesn't work the way you might expect, such as with common acronyms like HRT (doesn't map), ADHD maps but ADD doesn't, "plan b" won't map to levonorgestrel if you put it in quotes but it will map if you don't, etc. If you're going to rely on ATM, I'd recommend checking each term in the search details to see if it's actually doing what you expect. I use an admittedly unsophisticated example to show people how MeSH can focus a search. I ask them to look for articles on high blood pressure and a low salt diet. The difference between "high blood pressure AND salt" (yes, I said it was unsophisticated, but you know how some people search...) and hypertension/diet therapy/major heading AND sodium chloride is pretty startling. Personally, I think librarians and super duper savvy searchers are the only ones who really use MeSH because of the various reasons people have listed so far. I mention MeSH and explain it a bit, but I also know that the average user is going to keyword the heck out of things anyway because that is how Google raised us. That is why I make sure I always point out the Search Details box to show them how the system is thinking so they can adjust their search accordingly if the system misunderstands our intentions.

I find the MESH tree structures and scope notes very useful when I get too many articles, strange results, or no "older" citations when using a new concept. The tree structures are invaluable when I want to make sure that one explodes and includes my concepts. Gynecologic Surgery is an entry term for Gynecologic Surgical Procedures, which includes hysterectomy, colposcopy, etc. Good info, here! [...]

My favorite wonky mapping example (long ago fixed, so no longer useful for
teaching!) was when typing in "heart attack" mapped to "salaam attack" but
NOT myocardial infarction!

I agree, watching how things map or work in the Search Details box is
important.
Until a few years ago, if you typed white coat hypertension in the
PubMed search box, the Search Details showed you it was searching
Caucasians for the word white.
This used to be my favorite example to show people the usefulness of the
Search Details. Now that it has been fixed I am always looking for
another good example of wonky mapping.

I agree with Julie and would add that you should check the details to see
what MeSH terms PubMed has automatically matched your keywords to.
Sometimes it is great but sometimes it takes you in a direction you do not
want to go. Do a search using *Nursing* as a keyword and check the details.
Are all the MeSH terms the topic you wanted to search for? Do a search for
"insulin pump," note the number of results. Then do a search for "Insulin
Infusion Systems"[Mesh], note the number of results. Then run ("insulin
pump" OR "Insulin Infusion Systems"[Mesh]), note [...]

I would argue that automatic term mapping is what makes PubMed so much better than other search engines.

In a lot of searches, I find that my basic search results are better than my MeSH search results, but there are some for which MeSH is absolutely essential.

To me, one of the biggest disadvantages of MeSH is that the results won't include the very newest articles (those that are not indexed for Medline yet). [...]

I am old-school, admittedly (been a librarian nearly 20 years), but I find
MeSH extremely, extremely helpful. Controlled vocabulary has some real
benefits. It is hard to teach sometimes, but well worth the effort.
Controlled vocabulary is what makes PubMed so much better than other search
engines.

I'm just wondering if you notice working with mesh database on pubmed is just annoyiong and time wasting;

we usually work with non controlled kaywords on pubmed and it's fine , it's working well... i feel embarrassed with

my patrons wasting time explaining mesh but at the end results aren't satisfying ... [...]

I was trained to use the | pipe rather than OR. As to why they didn't suggest that may be that they don't think in complicated searches.

I've been using Google and Google Scholar for years and never had a problem
with lower case OR! And isn't it odd that Google Scholar Help didn't
suggest that, and instead said they don't support this type of searching?

long complex search for a systematic review from PubMed to search for grey

On June 3, AHRQ's National Guideline Clearinghouse(tm) (NGC) announced its revised criteria for inclusion of clinical practice guidelines. The new criteria reflect the Institute of Medicine's definition of a clinical practice guideline provided in its 2011 standards-setting publication, Clinical Practice Guidelines We Can Trust. The two main changes to the NGC inclusion criteria are that the guideline:
