Hi Diane,
You have a lot of stipulations for a dog. In any case, I have no idea
where you can find a dog like you describe: a puppy from an "oops"
litter that's like your previous dog and costs $100 or less and is perfect.
I always recommend rescue. The reason for the fees and adoption forms
are to make sure the dog is getting a good home and doesn't come back
into rescue. I did Border Collie rescue for 15 years and I can tell you
that many homes I visited were not adequate for the dog in question,
hence the paperwork and home visits. Being a foster home is expensive,
hence the fees.
