Last year, the state changed the 12 state forms including IEP's. They
created the forms in Microsoft Word, but they were intended to be
printed and then either typed on or written on. Shortly after the
state released the forms, I modified them so special services could
actually type in them on the computer.

Recently, two staff members have left Winooski School district and
requested the forms that I modified.

If anyone else is interested in these forms, please let me know and
I'll e-mail them to you or possibly host it on our web page if there
is enough interest.
