I just received a couple of boxes of the numbered rectangular cardboard
foram mounts from Fisher Scientific.  These mounts were originally made by
Curtin Matheson Scientific, which has now been taken over by Fisher.  The
original Curtin part numbers may be used when ordering from Fisher, adding
the prefix 22- before the part number.  They also carry the mounts with 1-4
round cavities and the glass/aluminum mount holders.  Price of the mounts
