Annals of Surgery was not on their list either, but the Medical College
of Ohio Library has a wonderful site.  It contains tons of links to
Instructions to Authors, and often this can also get you to the home
page of the journal and publisher.

Someone had posted this address on this list several years ago and it's
one of the most useful things ever.  Finding author instructions used to
always be a pain, but now...hooray for the Ohio crowd!!
