Medlibbers,

 

One of my clients is working on a quality project 

to improve our consents.  She is looking for

samples of generic consents for Consent to 

Operate and Render Other Medical Services.

 

If possible, would you please email me directly

any copies of your hospital consent form.

 

Any material you send she will keep the 

specific hospital confidential, or you can

remove the hospital name and city if you prefer.
