Hello!
I received a request from a nurse to get her a book from the National
Library of Medicine. I've ordered the book, but I told her she'd need
to sign an agreement about returning the book, paying fines, shipping
costs (if needed), etc.
We are a small library and have no such agreement--I was hoping one of
you might have something similar that I could use as a guide to draw
one up.
