Most tests are only available from the publisher-which is probably why you
haven't received any response. I consulted our 1994 edition of Tests in
Print and found that the test can be ordered from the following publisher:

Stoelting Co., Oakwood Center, 620 Wheat Lane, Wood Dale IL 60191

Looks like the cost in 1994 was around $44.00 for 30 sentence completion
forms, 30 record forms, and a manual.
