Does anyone have a copy of the Bloom-Wallace Sentence Completion Survey:
Adult?  It is designed to reveal subject's attitudes towards factors in
daily living.  The ETS tracking number I have is TC011048 but have been
unable to come up with it.  I really need it faxed to me if at all possible.
I will be more than glad to send you the amount it costs anyone to fax it to
