Hi Medlibbers, 

My patron is looking for a day to day assessment/questionnaire/tool for nurses to track inpatients fuctions such as eating, ambulating, pain, and how the patient is progressing in general.  There is a similar form that tracks monthly called SF-36.  I have done searches in CINAHL and MEDLINE, but may not have the search strategy set up correctly.  Has anyone ever searched on this and can point me in the right direction?  The patron, a physician, says there may not be such a tool.
