That makes sense that the problem may be updates headache rather than security. Zotero is constantly updating its Windows executable and that may be difficult on IT management.

We did stop paying and move to only the free services (Zotero & Mendeley) and EndNote Online, to which we get extended access as part of our Web of Science/Clarivate subscriptions. But we no longer ask for the plugins to be installed by IT. It wasn't so much security concerns as update headaches. As far as I know there have been no security concerns related to installing any of them on individual computers, just the problems of having it in the base image for all the computers on campus. (University, not hospital, so our security issues are not as stringent.)
I now teach citation managers as software that the students and faculty must choose and install for themselves, and otherwise I recommend that they use the saving tools within our various databases, email to themselves, and/or use web-based citation formatters like the tools in our databases, Google Scholar, or Zotero Bib (zbib.org).
