I have been searching unsuccessfully for a good 1 or 2-page cheat sheet that
provides a quick overview of the types of citation formats described in
depth in NLM's publication Citing Medicine
(http://www.ncbi.nlm.nih.gov/bookshelf/br.fcgi?book=citmed), 2007, rev.
2009.  Alternatively, I'd love to find a DIY citation generator like
KnightCite (http://www.calvin.edu/library/knightcite/) but with templates
for Citing Medicine.  

So far I have found:

I have also found a few quick sheets like the one above that reference
earlier editions of the NLM guidelines (1991, 2001) and also AMA and
Vancouver style, but I'm really only interested in any that match up to
Citing Medicine.  Any suggestions much appreciated!
