We are still looking for this reference, and it is proving to be mighty elusive. It appears as a citation in varying forms in a number of other articles, but the only confirmation of its existence is in Worldcat (below). Even its own university library catalogue doesn't show it.
A long shot, I know, but perhaps there's a copy hiding out there somewhere?
