I'm looking for any information on the proper method for measuring
intima-media thickness on a carotid duplex scan.  Specifically,

-- what are the indications for this measurement and the frequency of its use
-- what is the accepted method of measuring (near vs. far wall preference,
location within the vessel, a mean vs. single measurement)
-- general opinions on its use
