 Dear Gail, 

 I am not making any promises but try using two ICD.9 codes for dialysis
access:

1. Renal dialysis                   V45.1 
2. Renal dz, HTN, FMH, Thromb.etc   403.91

note: I believe that to use a code beginning with "V" that another code
explaining the dz must be included?? I am sorry to be so vague but we are
all flailing about out here with this medicare and coding nightmare. Let
me know how it works out for you.
