I work with experts in the field of pelvic congestion Dr Maureen Kohi UCSF
and several at Stanford using IVUS.
Since you need to know the health of the IVC and iliacs IVUS is the best
choice since you cannot close an gonadal vein off the Renals to the pelvis
if that is their only collateral.
I have an extensive vascular ultrasound background with abdominal and
imaging the gonadal vein is going to be difficult. I also work with several
private offices that have IVUS.
Perhaps there are hospitals and private offices doing ultrasound for pelvic
congestion. I know you will most likely have to do transvaginal with color
flow Doppler.  Here is an article

   - ovarian vein >5-6 mm (positive predictive value of 71-83%)
   - may show multiple dilated veins in the adnexa with reversed venous
   flow on color Doppler, especially after Valsalva maneuver
