After much searching, I discovered that this publication is  put out by the
American Board of Professional Psychology, but its  office did not offer
much hope of finding an older issue.
Here is the citation I have:

Butters, MA and Butters NM (this is apparently  Nelson Butters )
Contributions of neuropsychology to the diagnosis of dementia.
The Diplomate vol 14 : 5-7  (1993?)

Since the 2001 volume was volume 20, it seems to me that 1993 would be vol
12, but ABPP says vol 14 was 1994...
