The best suggestion that I could come up with is on page 214 of the 5th
edition, Personal Communication (section 3.102).  However, would you want to
cite a particular patient record due to patient confidentiality?

I had a question on how to reference/cite Patient Records in APA format.  I
couldn't find anything in the APA Publication Manual.  Any ideas?
