Even clients who are closed with VR should have a signed release that allows you to share information for a certain period of time. Since your agency and AHS are both covered entities under HIPPA, we should also be able to share information on other clients that are being served under the grant, but as I mentioned at the meeting, I will draft guidelines so that everyone is on the same page in regards to issues of confidentiality. At this time, we are not asking for any other information that has not been provided in the past as part of your grant agreement. After thinking about it, I realized that technically I can't give names of clients who are not open, if I don't have a release signed to give you information. So for now I will only be giving info. on JOBS clients open (or in referral status) to VR. HOwever, I would like to discuss this issue further, so that all the programs are submitting the same data. When I send the disk, should it include kids in JOBS, that are not open to VR? Up until now, I have not included them, because that's what I was instructed to do. At the last meeting I thought I heard something including all kids in JOBS.

I have volunteered to assist Glen with the collection of Closure Indicators summaries this year!~.
I am writing to enlist your support to complete the JOBS Program legislative update for Fiscal Year 2004. As you know this data, combined with closure data from previous years continues to help us strengthen existing JOBS sites and expand impact of this program. Please take a few minutes to complete the attached JOBS Program Closure Indicators Questionnaire for each young person who was successfully closed from October 1, 2003 through September 30, 2004. The vocational rehabilitation counselors assigned to your program can give you the names of individuals who have been successfully closed during this time period.
Please use this form for each individual, rather than creating a new form. Please forward completed forms to Jan Sherman by December 10, 2004.
