Is is appropriate for the physician performing the pseudo injection to have 
the "practice" bill for the injection and u/s guided repair componants, 
thus leaving only the diagnostic exam to be billed in the vascular lab?  or 
do all 3 CPT codes have to originate from the vascular lab?

Appreciate any feedback.
