With a positive outpatient DVT we notify the referring doc and usually admit them; sometimes we're directed to take the patient to ER in a wheelchair--tedious for the patient that way, but we don't decide these things. Occasionally a doc says to send the patient home, even after we try gently to suggest that the thrombus appears rather fragile, etc. All we can do then is advise the patient not to play racquetball.
