We did something similar to what Ann Marie described and it was approved by 
legal as well.  The private practice used encounter forms for their 
charging/orders so we utilized the back of each by have them printed with the 
vascular lab "encounter" form.  It was easy for the docs to flip over to order 
whatever study they needed.  We kept copies in the lab and the medical 
office kept the originals since their billing info was on it too.  

By the way, I understood that emails were acceptable as an order if it was 
from the ordering doc.  Can anyone verify that or did I dream it?
