We generate the whole report, print a copy, stamp it "Preliminary
Report" in large red letters, then the physician comes along and signs
it and it flows to the electronic medical record.

We have been asked to evaluate how other facilities are documenting
preliminary reports in inpatient charts. So I came here.
What are you documenting the full "findings", or just that the exam is
completed, or anything in between?
