We only use the G0365 code for patient's first vein mapping for AVF placement (perhaps I misunderstood your statement). We never use it for the first scan of the functioning AVF (also use 93990).

Yes. I use the G0365 when scanning a patient for a brand new fistula. I only
use this for my medicare patients and I only use this for their first AVF.
After that, and someone out there please let me know if I am mistaken, I use
the 93990 for follow ups usually with the 996.73 code. I use 93970/93971 if
they will need another AVF or for patients that are nonmedicare vein maps. (I
can't remember the last time I did a v. map thet was not medicare,though)
Seems like it shouldn't be this difficult, doesn't it?
