We use V72.83 and the code that corresponds to the diagnosis for which the surgery is being done, ie 440.21 if for atherosclerosis with claudication, 585.6 if for a dialysis fistula because of renal failure.  Can't use the v code alone.  We don't use modifiers and I have not seen any denials.

What ICD-9 code are you using for your preoperative vein maps? Is everyone 
using the V code, if so do you have to include a modifier? Is anyone getting 
denials?
