I like this response, as well as those from Ann Marie and Barbara. We're supposed to be good at providing diagnostic images AND to be smart when we do it. As someone who is this week immersed in administering scanning performance tests, I can tell you that trying to have everyone do a practical test for ARDMS would be like digging another Panama Canal with a spoon. So computer-administered knowledge tests are about the only way to try to insist on some cognitive competence, at least.

It's true that registry is not universal, but my impression is that nearly all hospital-based vascular labs are insisting on it, and at my place two general ultrasound techs were fired for not getting registered, wow. A bigger problem for quality is the physician office setting, I think. What will nudge quality upward is Medicare (and therefore all insurance companies) not reimbursing unless a registered sonographer does the study. It's overdue to insist on that.

I took the ARDMS exam twice: once in 1985 for real, and once five years later to see if I was teaching in the mainstream. (They allowed me to do that then and left me out of the statistics; they naturally wouldn't do that now. We were all younger then.) Both times I took the exam it seemed reasonable to me, something everyone should pass to be allowed near a patient. In any exam like that, there are questions you know are cockeyed and will miss, would like to argue with someone about. You just let them have it and move on. I also know about making up test questions, and ARDMS does a good job of winnowing out the dog-questions and keeping good ones in the test bank.

Apropos of this and other things going on these days, I think it would be good for people not to have so much fun being angry.

Why wait for every other field? I think it would be great if our
field set a great example and every other other field of medicine
wanted to be like ultrasound. I think whoever is proactive and sets
the standards will stand out more and be more successful, compared to
those who sit back and wait to see what everyone else is doing.
