There are several "it depends" in this one, starting with differences based on the surgery(ies). For example, was it an axillo-femoro-femoral bypass graft (one graft, three anastomoses, two being end-to-side and one side-to-side) or an axillo-femoral AND femoro-femoral bypass grafts (as in two separate grafts). I probably could assume a duplex evaluation (vs physiological), but history has taught me that this could be a mistake.
Procedure coding depends also on your facilities P&P (surprisingly important - e.g., where the difference between complete bilateral and unilateral limited) - wisdom dictates that the entity submitting claims would have these defined. As such, several possibilities exist.
May need to take this one off-line.
