Larry and I always left it as not anonymous so that we could use the 
attributes, but we maintained confidentiality and treated the surveys as 
anonymous when we downloaded the data. Having them not anonymous is 
actually useful. Sometimes students will email asking you to delete 
their responses and let them start over, because they got mixed up and 
filled out an evaluation for one course thinking it was another, or they 
thought better of some comments that they wrote.

Andrew's idea of asking the survey taker to provide the information 
would be less than reliable in this scenario.
