I am looking for an article or anything (at this point) that has the ACTUAL SURVEY TOOL in the article. This is the WilsonSims Fall Risk Assessment Tool for Behavioral Health Units. I've looked, but cannot find it. I find tons of references to it, but not it. Can anyone help? Thanks so much and Happy Thanksgiving!

PS: the "article" that appears in the Journal of the American Psychiatric Nurses Association 2014, entitled "Gravity: It's not a Suggestion-It's the Law!" is NOT a research article; it's a blurb about a conference session. I'm at the point where if anyone's hospital uses this tool, could I have a copy of it? <sad trombone>
