Below is a summary of responses received to the question regarding an
electronic template for document delivery on the OPAC:
FIU uses a template for ILL requests, another for requests for purchase
and another for intercampus loan.
Another respondent reported using a template to request books for
purchase, request photocopy of papers, reference questions, online search
requests, etc.
A system (by SIRSI Corp.) used by the Anderson Cancer Center allows
registered users to place holds on items that are charged to another user
without library staff intervention.
One response stated that the topic was discussed at MLA in 1992.
The Medical Center of Delaware has "useforms" that are used for
interlibrary loan and online search requests.
Battelle Seattle Research Center uses a electronic form to request
borrowing and copying. They copied the paper form using a word processor.
