I am currently setting up a new vscular lab for a 2 physician practice in a
small town.  One of the surgeons has asked me to develope a vascular history
form to be filled out by all patients.   He especially wants the "indications"
seperate from the "risk factors" (i.e. claudication vs. diabetic etc.).  Does
anyone have any suggestions for where I could find a "fairly complete" list of
these things?  Any specific text with sample forms would be great to get
started.
