What common criteria are used to interpret intraoperative carotid scans?

What are typical peak systolic/end diastolic velocities in patients who have 
significant residual disease?  How does one differentiate between spasm and 
stenosis?  How do the velocities vary in patients with contralateral carotid 
occlusions?
