As far as Carotid orders, all carotid studies after hours have to go through
the neurologist on call first. Eye twitches, ect are surely not a reason for
a carotid. Some are told to make appointments for during the day and many
inpatients are put off till the next day as a result of this policy. This
does save alot of unecessary call backs. You still get those vague calls
"Arterial Stats are usually done only for the patients convenience" as told
by one Vascular Doc I know. If a patient has an acute cold blue, not
swollen, leg , then they should be going for an Angio. Unfortunately, ER's
have become a means for HMO docs to get tests without getting prior autho.
I believe you need a strong Vascular Surgeon who is your Medical Director to
develope guidelines for after hour exams.
Venous studies were listed as one of the most over ordered studies done. I
think it said it beat out chest x-rays!
It woudl be interesting to develope a Nation wide research study into the
reasons for "Stats" and the outcomes.
