This group is amazing ! After testing today, I checked the form and had 20 responses. Thank you so much. If anyone else is interested in this information, please let me know. 
Quick summary is that 81% of respondents are 1:1 at the elementary level and the remaining 19% have a hybrid which equates mostly to shared devices in K and 1 or k-2 and the remaining have 1:1. 
This will be super helpful to us. Thank you again!

We continue to advocate for 1:1 in our elementary schools in Burlington ( in the upper elem grades, in particular) and need some support from other districts in the area to know what this looks like in neighboring districts and beyond. 

Lack of 1:1 devices becomes particularly apparent during the testing window . 

If you are able to complete this brief form, that would be greatly appreciated. If not but you are able to respond with a quick Yes 1:1  or No or Hybrid (with grades and devices) and your district, that would be amazing as well!
