I just saw this same issue on Reddit using Go Guardian. The link that Ryan listed above has the same settings that fixed the issue, Network Prediction and pre fetch often load pages first that may be blocked and then the entire search gets blocked.

I've reached out to Securly, and they've informed me that this has to do with a change Google has made in how it presents the Summary bit at the top of the page.  If the Google Auto Generate Summary that is shown from the top gets that info from a blocked site, it will block the search entirely.
