You're a little over thinking this whole rebuilt Lance thing.  Before his
cancer he was too heavy (i.e. he had too much muscle where it wasn't
needed), it's hard to loose muscle, without it turning to fat.  His chemo
caused muscle atrophy, so he was, in essence starting from scratch and
could choose which muscle groups to build up.

His ability to sustain an incredibly high cadence is the result of his
unmatched fitness and he probably has an unusually low heart rate a la
Indurain.
