As you know this data, combined with closure data from previous years continues to help us strengthen existing JOBS sites and expand impact of this program. Please take a few minutes to complete the attached JOBS Program Closure Indicators Questionnaire for each young person who was successfully closed from October 1, 2003 through September 30, 2004. The vocational rehabilitation counselors assigned to your program can give you the names of individuals who have been successfully closed during this time period. Please use this form for each individual, rather than creating a new form. Please forward completed forms to Jan Sherman by December 10, 2004.
