Thanks Bill, That isn't what we do here but I was curious about a scenario like I mentioned. Actually our surgeons typically order exercise for all pts who are able to walk and have exercise symptoms and if we find a reason not to exercise we will consult with the ordering surgeon. The order must come from the "treating physician." Internal protocols do not meet that criterion. In order for your medical director to write the order, he/she would need to be formally consulted to see the patient in which case they also become the treating physician. But you look at your protocols and it states that you should perform treadmil testing in this case (assuming no cardiac or other contraindications exist). You must have a order from a health care provider, MD/PA/NP-C, perform that test, and bill for that test. On the other hand, if you want to do the exercise testing even without an order for it, you must only bill 93923. Do you all feel this is a noncompliance issue and that we should get the order to state 'with exercise testing'? Thanks.
