What about if you had an algorithm with your Med Director's signature of approval that states something like "perform treadmil testing if such and such conditions exist"?   ie A pt arrives with symptoms of claudication and you have an order for a "LE Arterial Doppler" and you find the resting pressures and waveforms are normal.  But you look at your protocols and it states that you should perform treadmil testing in this case (assuming no cardiac or other contraindications exist).

Shelly:  If the order does not state "with exercise" and you perform the PVR with exercise, and then bill for it, namely 92924, then this is a billing problem, i.e., fraudulent billing.  You must have a order from a health care provider, MD/PA/NP-C, perform that test, and bill for that test.  On the other hand, if you want to do the exercise testing even without an order for it, you must only bill 93923.  Pat Marques, RN, RVT, FSVU, Vascular Associates Laboratory, Baton RougeLA

The compliance people at our hospital have discovered we perform treadmill testing on 90% of our PVR studies although the 

'script' does not always say 'with exercise testing'.  Do you all feel this is a noncompliance issue and that we should get the order to state 'with exercise testing'?  Thanks.
