I agree, get a fax pronto.  At very least, call the provider and then
document that you conversed with him/her.  Document, document, document
(this from a guy who hates paperwork).  Keep this in the back of your mind
(I can hear Frank chuckling):  without sufficient documentation your actions
could be construed as a fraud.  If you defraud the feds and get caught, your
life is over.  Don't believe me?  Call a federal prosecutor and ask.  I went
to an SVU sponsored conference that dealt with issues such as this one.  A
federal prosecuter addressed the audience and made a believer out of me.
