It's been a while since I reviewed the entire paper but your right - there are a lot of things that alter velocity in an extremity artery - with volume flow being the primary one.  There are several things that affect flow to the extremities and if the flow changes, so will the velocity.   This is why I really don't look at the absolute PSV in a peripheral vessel but was wondering what others thought.

Does the paper mention anything about cardiac output? If the pump isn't working well, the velocities will be lower due to decreased perfusion.
