SurveyMonkey can limit one response per IP address.  A problem with this approach is that it will not allow two different people to use the same computer.

This e-mail may contain information protected under the Family Educational Rights and Privacy Act (FERPA). If this e-mail contains student information and you are not entitled to access such information under FERPA, please notify the sender. Federal regulations require that you destroy this e-mail without reviewing it and you may not forward it to anyone.

Hi all,

I'm looking for a survey tool that will only allow one response per person yet still guarantee anonymity.

As far as I can tell in Google Forms, there's no way to ensure that each respondent only answers the survey once.

SurveyMonkey's email collector allows you to send a unique link to each email, yet it also allows you to track responses by email -- thus negating the anonymity.

Any suggestions?
