In response to Tanya's references to surveys, I'd like to share my
recent experience.

This hospitals' Dept. of Continuing Medical Education sent out a
one-page three-question needs assessment survey by fax to our medical
staff a few months ago. They received 3 responses.

I sent out a needs assessment survey to the same group (actually, to a
few less people). My survey had about 16 questions, was done on Survey
Monkey, and offered a $25 gift certificate to Amazon to one respondent
to be chosen at random. As many as possible (about 80%) were
distributed by e-mail. Those MD's for whom I had no e-mail address at
all were given a letter in their mailbox asking them to go to the
Survey Monkey questionnaire. The memo stated that "others told me the
survey took them 5 minutes to complete."

I got a total of 54 usable surveys returned, some of those were
actually from the people who got the memo on paper in their mailbox.
So, the point is that HOW one presents a survey makes a great deal of
difference.
