Subject: Perf worksheet

Does anyone have a lower extremity worksheet for mapping perforator veins?  It seems cumbersome to write it all in a free text field, and can lead to some confusion.  I would like to find a great way to document location by measurement from the tibia and the malleolus.  Does VNUS have such a worksheet?
