Doesn't it essentially do that with any subclavian steal?
Could there be a distal ICA occlusion or high grade stenosis, or could
there be a distal subclavian aneurysm or an AVM in the arm that was
lowering resistance enough to steal from the ICA?   Could you try
putting a BP cuff on the arm to occlude the arm outflow and see if the
ICA goes antegrade?

Good point Jack... As I say everything is possible... it is still a
pretty 
grave concern (to me at least) that the brain or part of the brain would

"give up" flow for a skelettal muscle.

Had something similar, HD access tied to carotid instead of subclavian.
Patient had "syncopal" episode every time they dialized.  I wonder why?
Not every plumber knows where the pipes are I guess.

What would concern me with such scenario is what is actually going on in
the
brain of that person for flow to be diverted from the brain to the
arm...
(assuming the graft was done to revascularize the arm). Anything is
possible
in hemodynamics but I guess a thorough clinical evaluation is clearly
needed
here..
